Welcome to Restore Behavioral Health ("Restore," "we," "us," or "our"). These Terms and Conditions ("Terms") govern your use of our website, located at restore-bh.com (the "Site"), and your relationship with our counseling practice, with locations in Norman and Oklahoma City, Oklahoma, and online. By accessing or using the Site, or by becoming a client of Restore, you agree to be bound by these Terms.
1. Not a Substitute for Emergency Care
Restore provides outpatient counseling and behavioral health services. Our Site and services are not intended for use in a medical or mental health emergency. If you or someone you know is in crisis or experiencing a life-threatening emergency, call 911 or go to your nearest emergency room. You can also reach the 988 Suicide & Crisis Lifeline by calling or texting 988.
2. No Doctor-Patient Relationship via the Site
Use of this Site, including submitting a contact form or scheduling request, does not establish a therapist-client or provider-patient relationship. That relationship begins only once you have completed intake and been accepted as a client by one of our providers.
3. Eligibility
You must be at least 18 years old to use this Site on your own behalf, or the parent/legal guardian of a minor for whom you are seeking services. Services for minors require the consent of a parent or legal guardian as required by Oklahoma law.
4. Scheduling, Cancellations, and Fees
Appointments may be scheduled through the Site, by phone, or by email.
We require [X hours/days] notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee as disclosed in your client agreement.
Fees for services, insurance, and payment policies will be disclosed to you separately in your client intake paperwork.
5. Telehealth Services
For clients receiving services online, telehealth sessions are conducted through a HIPAA-compliant platform. Telehealth is not appropriate for all situations, including certain emergencies. Your provider will discuss the risks, benefits, and limitations of telehealth with you as part of informed consent.

7. User Conduct
You agree not to misuse the Site, including attempting to gain unauthorized access to our systems, interfering with the Site's operation, or submitting false information through any forms.
